Doses of therapies that comprise guideline directed medical therapy
Appropriate use of guideline recommended therapies. This will include doses of beta blockers, ACE inhibitors, ARBs, ARNI, SGLT2i, ICD use.

- Seen within the Yale New Haven Health System
- Diagnosis of heart failure
You may not qualify if:
- Age\<18
