Eligibility Criteria
Patients of Martha Morehouse General Internal Medicine Clinic at the Ohio State University Wexner Medical Center
You may qualify if:
- Patient of Martha Morehouse General Internal Medicine Clinic at the Ohio State University Wexner Medical Center
- Medicare A/B is listed as primary insurance
- Patients 65 years old or older
You may not qualify if:
- Patients less than the age of 65 years old
- Patients with Medicare Advantage plan
- Physician Population
- Physicians of Martha Morehouse General Internal Medicine Clinic at the Ohio State University Wexner Medical Center
