Brief Summary
This prospective observational study aims to compare the diagnostic performance of non-contrast magnetic resonance angiography (NC-MRA) and carotid Doppler ultrasonography in the evaluation of carotid artery disease among patients with ischemic cerebrovascular stroke or transient ischemic attack. A total of 200 adult patients presenting to Sohag University Hospital will undergo both imaging modalities. The study will assess the agreement between NC-MRA and carotid Doppler in detecting and grading carotid artery stenosis according to the NASCET criteria, with the goal of evaluating the clinical utility of each technique and optimizing imaging strategies for the assessment of cerebrovascular stroke.

Outcome Measures
Primary Outcomes (1)
Internal Carotid Artery Stenosis Grade (according to NASCET criteria) assessed by Non-contrast Magnetic Resonance Angiography and Carotid Doppler ultrasonography.
Internal carotid artery stenosis grade in patients with ischemic stroke or transient ischemic attack determined according to the North American Symptomatic Carotid Endarterectomy Trial (NASCET) criteria using non-contrast magnetic resonance angiography and carotid Doppler ultrasonography. The stenosis grades obtained by the two imaging modalities will be compared.
At the baseline imaging visit (within 7-14 days of symptom onset).
Study Arms (1)
Patients with Ischemic Stroke or Transient Ischemic Attack
Adult patients (≥18 years) with clinically diagnosed ischemic stroke or transient ischemic attack who undergo both carotid Doppler ultrasonography and non-contrast magnetic resonance angiography for evaluation of carotid artery disease. The findings of the two imaging modalities will be compared for the assessment of carotid artery stenosis and atherosclerotic plaque characteristics.
Eligibility Criteria
Adult patients (≥18 years) presenting to Sohag University Hospital with a clinical diagnosis of ischemic stroke or transient ischemic attack (TIA) who are eligible for magnetic resonance imaging and carotid Doppler ultrasonography.
You may qualify if:
- Adults aged 18 years or older.
- Clinically diagnosed ischemic stroke or transient ischemic attack (TIA).
- Imaging performed within 7-14 days of symptom onset.
- Hemodynamically stable and suitable for magnetic resonance imaging (MRI).
- Able and willing to provide written informed consent.
You may not qualify if:
- Hemorrhagic stroke.
- Previous carotid artery stenting.
- Contraindications to MRI, including:
- Non-MRI-compatible pacemakers or other MRI-incompatible implants.
- Cochlear implants.
- MRI-incompatible metallic orthopedic implants.
- Severe claustrophobia.
