Number of medication discrepancies identified and the number of medication discrepancies resolved by the pharmacist at discharge.

EXPERIMENTALDischarge medication services included: * Discharge medication reconciliation * Identification of medication discrepancies and resolution * Medication counseling using health coaching techniques with short term goal setting and follow up phone call

NO INTERVENTIONCurrent standard of care provided by nursing staff.

You may qualify if:
- Diagnosis of heart failure, discharged from any medical service
You may not qualify if:
- Unwilling to participate, discharged to nursing/rehab facility, non-English speaking, resident unavailable
