Outpatient pain clinic patients
You may qualify if:
- years of age.
- patient presenting to the UAB Highlands Pain Treatment Clinic for initial evaluation and treatment or subsequent follow-up evaluation and treatment.
You may not qualify if:
- \<19 or \>100 years of age
- a current prisoner
