Eligibility Criteria
You may qualify if:
- dementia diagnosis
- advanced cognitive impairment
- feeding problem
- age \>= 65
- surrogate decision maker
You may not qualify if:
- feeding tube decision made
- hospice
- BMI \> 26
- major psychosis or developmental delay
