Age GroupsChild (0-17), Adult (18-64), Older Adult (65+)

You may qualify if:

  • Patients with a history of asthma

You may not qualify if:

  • Current asthma exacerbation or recent asthma exacerbation, requiring hospitalization
  • History of smoking
  • Debilitating systemic and/or life-threatening diseases.
